The 5^th annual Spring Slam volleyball tournament hosted by the Christian Student Center will take place on April 24th, 2009.  This tournament will be a six-on-six, double elimination tournament.  Teams can have a maximum of 8 players.  Simply fill out this form, sign the waiver on the back and return it to the Christian Student Center by April 17^th, 2009. Agreement, Waiver, and Release

In consideration for being permitted by the Christian Student Center to participate in the      tournament, I hereby waive, release, and discharge any and all claims for damages resulting in personal injury, death, or property damage which I may have, or which may hereafter accrue to be, as a result of participation in the volleyball tournament, even though that liability may arise out of negligence or carelessness on the part of the Christian Student Center knowing those risks I hereby assume those risks.  I agree to indemnify and to hold the Christian Student Center free and harmless from any loss, liability, damage, cost, or expense which that may incur as the result of any injury, death, or property damage that I may sustain while participating in the   volleyball tournament.

I HAVE CAREFULLY READ THIS AGREEMENT, WAIVER, AND RELEASE AND FULLY UNDERSTAND IT CONTENTS.  I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND THE CHRISTIAN STUDENT CENTER AND I SIGN IT OF MY FREE WILL.
